Employment Paperwork

Welcome to Star Island Corporation. We are looking forward to a fantastic season and we are glad you are with us!

To be employed on Star Island this summer, you must provide the following information to our Portsmouth office. This paperwork should be submitted to us as soon as possible but no later than one month before your start of employment. We must have all of your paperwork completed, signed, and in our possession before you are permitted to be employed by us.

  • A completed and signed W-4 form (download form)
  • A completed and signed I-9 form (download form)
  • A completed and signed “Star Island Employee Health Form” which does not require a medical professional’s signature-please follow the instructions on the form for submitting your medical information. (download form)
  • A completed and signed Sexual Offender Record Information form (download form)
  • If you live in Massachusetts (download form) or Maine (download form) please complete the state specific tax forms.
  • If it was indicated in your employment agreement that you are to undergo a more detailed background check, follow the steps in the Background Check Instruction Form. (download form)

Voluntarily you may complete the EEO Demographic Form-SIC encourages you to fill this out so we can report accurate demographic data to the Equal Employment Opportunity Commission. (download form)

Optionally, you may complete, sign and submit the “Paychex” Direct Deposit form. Be sure to include a voided check, bank letter, or specification sheet, as specified on the form. (download form)

If you are required to submit an I-9, SIC must physically review your identification. If you are departing to Star Island Monday through Friday, you will be required to come to Star Island’s Portsmouth office 90 minutes prior to your boat departure in order to submit your completed I-9 form and show proof of work eligibility. If you are traveling to Star Island on a Saturday or Sunday, you will be required to do the same thing on island immediately upon your arrival. Star Island’s Island Clerk will process your paperwork. If you fail to bring your identification with you, it will be your responsibility to ensure your identification is received and verified by SIC within three days of your start of employment. If this is not done, you will be required to stop work in which case you may be required to leave the island to accommodate any replacement that may be hired to fill your position. No one can work for Star who does not submit the required identification.
